HealthPoint offers a variety of payment options for
patients. If you have no health insurance, you may qualify
for discounted fees based on income and family size.
For example, we charge only $30-$40 for office visits
if your annual income is:

Under $20,799 for 1-person household

Under $27,999 for 2-person household

Under $35,199 for 3-person household

Under $42,399 for 4-person household
If
you would like to have your financial situation evaluated
for the discount plan, we will interview you during
your first visit to the center. Please bring with you
the following for all adults in your household:
- Income
Tax Forms or W-2 Forms
- Two most recent paycheck
stubs
- Most recent rent receipt
If
you have Medicare, HealthPoint
does NOT charge the $100 yearly Medicare deductible
to any patient. Your 20% co-pay for office visits
is NOT charged if your annual income is:
- Under $20,799 for 1-person household
- Under $27,999 for 2-person household
- Under $35,199 for 3-person household
- Under $42,399 for 4-person household
- Under $49,599 for 5-person household
If
you have Medicaid, we welcome
patients with medical
cards and children covered
by KCHIP. If you have
insurance, we take most major
health insurance
plans,
including
Aetna
Golden Medicare,
Anthem Kentucky
and Anthem Ohio, ChoiceCare,
Humana, United, Prudential
and Cigna.
